Purposes, Parameters, Qualifications, Activities and Performance Indicators

for the position of

Nursing Home Transition Coordinator


  1. Purpose of Position:. To provide personalized, consumer driven services to persons leaving a nursing home to return to the community services include assessing the individuals eligibility, collecting pertinent documentation, information & referral, peer counseling & support, education and advocacy, and independent living skills training and more, while advocating for the rights of the individual to make their own choices. This position also requires an understanding of the principle of the Independent Living (IL) philosophy, and a genuine desire to carry out this philosophy.


  1. Position Parameters: Reports; Reports directly to the County Manager; Full-time, Full Benefits, Travel and Expenses Reimbursed.


  • Minimum Qualifications:


  1. Bachelors Degree in a field directly related to the operations of RAMP; or
  2. Three (3) years of equivalent experience and understanding of non-profit or direct service programs; or
  3. Personal experience with a disability and/or working with persons with disabilities:
  4. Excellent written and verbal communication skills and the ability to work as a team player;
  5. A belief and commitment to the Independent Living (IL) Philosophy and a vision for seeing it implemented for all persons in our four county catchment area;
  6. Self-Motivated and creative in problem solving;
  7. Reliable transportation and a willingness to work evening and weekend hours, if needed.
  8. Ability to pass the Department of Rehabilitation Certification process.



  1. Job Activities and Performance Indicators (in order of priority):


A1.         Collaborate with consumers wishing to leave the nursing home to identify needs using a holistic approach and taking into account all areas of consumers' lives.

  1. Complete all paperwork required by DHS and develop an Independent Living Plan (ILP) with consumer's goals, objectives, and time frames for completion to assist in a smooth transition from living in the nursing home to returning to independent living in the community.
  2. PI. Provide encouragement, follow-up and support to Community Reintegration consumers and family members to ensure a smooth transition to community living.
  3. PI. Refer consumers to other community agencies or services providers to ensure appropriate supports for returning to community living.
  4. PI. Obtain necessary furnishings, equipment and/or modifications to ensure that needs are addressed.
  5. PI. Provide necessary case management and follow up after return to the community to ensure successful reintegration into the community
  6. Continually search for new housing opportunities within the service area.


 A2.             Actively serve as an advocate of the Independent Living Philosophy.

  1. PI. Pursue and remain abreast of advocacy issues related to equal rights for people with disabilities on both an individual and systems level.
  2. Maintain a high level of visibility within RAMP's four county service areas so that coordination and opportunities for persons with disabilities are at an optimum.
  3. Attend and conduct networking activities with other agencies, service providers and organizations to ensure that consumer needs are being met.
  4. Participate in statewide Transition Coordinator meetings and trainings and ensure internal team members are kept up to date on any changes to the program.


 A3.             Maintain enthusiasm and creativity in assisting consumers.

  1. PI. Meet with County Manager on a regular basis to discuss individual consumer services, and to review goals and objectives.
  2. PI. Continue personal and professional development through educational opportunities, attending workshops and conferences, participating in peer/case reviews, and by attending all staff/development meetings.


 A4.             Educate the community on what assistance and services are available     

                   through RAMP.

  1. PI. Market RAMP Community Reintegration services in collaboration with other RAMP staff through media releases, service fairs, radio and television interviews, advertisements, etc., as allowed by available resources.
  2. Remain in constant communication with all nursing homes in RAMP's service area to obtain referrals for the program.
  3. PI. Be available for community presentations on RAMP' s mission and services, the IL philosophy and the Community Reintegration Program.
  4. Collaborate and network with other for profit and non profit entities to promote RAMP and the Reintegration Program.


A5.             Maintain an active caseload and complete all necessary paperwork and documentation

  1. Contacts new referrals within RAMP's guidelines and provides follow up until needs are met and/or intake is scheduled.
  2. PI. Complete necessary intake information; consumer profile, release(s), rights and responsibilities, and when needed access documentation necessary for the provision of premium services.
  3. PI.
  4. Complete notes within 48 hours of service provision, and document daily services on the Service Reporting Form (SRF).
  5. Conduct required home visits within designated timeframes.
  6. Provide consistent, regular follow up with consumers and encourage them to increase involvement in their community.
  7. Complete MFP &/or CRP paperwork in a timely basis and submit to DRS for approval.
  8. In collaboration with key staff in RAMP's four offices, ensure a minimum of 15 individuals reintegrate back into the community annually.




A7.             Complete monthly paperwork and quarterly reports in a timely fashion as required.


  1. Make sure that all monthly CRP/MFP tracking forms are complete & submitted to the Finance Manager by the 10th of the month.
  2. PI. Make sure that the quarterly Community Reintegration report is approved by the County Manager, and submitted to the Services Director by the 5th of the month following that quarter.
