The Adult Day Health Programs Manager is responsible for developing and maintaining a robust Adult Day Health Program in our Huntley location. The position directs all aspects of planning and program development, and provides overall management of daily operations including supervision of staff and services to ensure efficiency of operations. A key element in the position is developing the program's growth by community outreach and marketing.

 

  • Leads program design and strategy aimed at creating a supportive environment that fosters ongoing, diversified wellness activities to clients and families. Revise and/or update existing programs or develop new programs to meet an ever-changing target population.
      • Maintain awareness of client demographics and any shifts in same
      • Solicit staff input as to client needs and interests, what works, what doesn't, etc.

 

  • Provides for and maintains a high client and/or caregiver satisfaction rate. Addresses issues brought up by dissatisfied customers with the mindset of turning the challenge into an opportunity for improvement.

 

  • Fields inquiry phone calls, providing information on available services, fees, and any available funding while screening for appropriateness for services. Guides interested and appropriate clients through the pre-intake process. Sets up and conducts intake appointments.

 

  • Serve as primary clinician for a memory care and behavioral health-focused assigned client case load, providing supportive counseling, education, skill development and problem solving assistance to clients on site and/or families via consultation.
      • Monitor success (or lack thereof) as to clients meeting their goals and objectives
      • Offer suggestions as to client needs and preferences
      • Update treatment plan to reflect status changes in client needs or preferences.

 

  • Acts as liaison between nursing, dayroom, clinical and transportation teams, facilitating and ensuring communication of operations and client needs.
      • Co-lead weekly dayroom staff meetings as necessary or requested
      • Attend transportation meetings as scheduled by transportation coordinator.
      • Attend weekly clinical staffings as requested by behavioral health to facilitate coordination of shared clients.

 

  • Strives to keep staff motivated and functioning as a team.
      • Provide opportunities for growth and development.
      • Arrange for team building exercises/skills training as needed.

 

  • Models desired behavior for staff. This may be through:
      • Periodic hands on service to clients such as personal care, behavioral interventions, or leading activities.
      • Personally utilize expected communication patterns and treatment of fellow staff

 

  • Participate and lead community outreach and marketing with the goal of program growth and census increase.

 

  • Collaborates with QA and Policy Manager to
      • Develop or revise policies and procedures
      • Maintain documentation standards
      • Remain up to date with accrediting standards/funder standards.

 

  • Collaborates with HR Manager to
      • Apprise HR of department staffing needs
      • Evaluate candidates to determine new hires for the department.
      • Document staff training
      • Complete staff evaluations within expected time frames (new hires within 30 days, then all staff annually)
      • Coordinate and monitor volunteers or interns specific to the day programs.

 

  • Collaborates with the Finance Manager to
      • Develop department budget
      • Recommend salary actions as appropriate

 

  • Collaborates with the Development Department
      • To ensure privacy measures are in place for all press or marketing endeavors associated with the day programs
      • Regarding target customer base and prospective customers
      • In setting strategic marketing goals 

 

  • Serve as final authority for
      • Day program admission and discharge decisions
      • Authorization of budget expenditures to be turned in to finance.

We are seeking an applicant who is a graduate of Associate Degree. Bachelor Degree in a Health Services field (Nursing, Human Services, Social Work, etc). Will consider job experience for the qualified candidate.

The ideal candidate will have four or more years working with adults 50 and over; minimum of one year supervisory experience.
