Our mission is to provide adults, caregivers and families with programs and services promoting an enhanced quality of life. Our programs allow individuals to remain in their homes for as long as possible, avoid premature nursing home stays, and minimize hospitalizations, significantly reducing the cost of care.  Our approach to program services is administered via a culture of empathy and compassion.

The Adult Day Health Programs Nurse utilizes the nursing process to manage the day-to-day nursing needs of persons attending the Adult Day Health Programs.

  • Conduct assessments and relay assessments of client's physical, psychological and social status.
  • Utilize nursing diagnoses to plan with client and/or family member.
  • Confer with drivers daily regarding clients' bus ride to program.
  • Handle medical emergencies and obtain prompt services of emergency personnel as needed.
  • Educate, monitor, and administer medications; supervise treatments for clients.
  • Contact family members, caregiver, or responsible party of any changes in client's status.
  • Display sound nursing judgment by seeking additional input and/or discussing complicated situations with supervisor or clinical director.
  • Implement nursing care plan.
  • Evaluate client progress

Minimum Education Requirement: Graduate of an accredited associate, diploma, or licensed practical nursing program.

 

Minimum Work Experience: New graduates considered. Preferred two years of experience in medical/surgical, mental health, home health, hospice, and/or gerontology.
